Please remember that this is your cataract evaluation. You will NOT be having surgery on your first visit.
Please fill out the paperwork you may have received from your optometrist in the RED folder. The paperwork can also be found under forms.
Please bring a driver with you since we will be dilating your eyes. You should bring your sunglasses with you as well.
Bring a complete list of your medications including vitamins and eye drops.
Bring your insurance cards and photo-identification. You will be expected to pay your co-pay at the time of your visit.
If you are a contact lens wearer, we would prefer that you not wear the contacts 7 days prior to your appointment. RGP wearers will require a longer period without contact lenses.
Dr. Batchelet asks that you start Preservative Free Artificial Tears two weeks prior to your appointment. You can also do warm compresses twice a day starting two weeks prior to your appointment.
